POSITION SUMMARY:The Neuroscience Coordinator under the supervision of the Senior Director of Neuroscience, is responsible for leading the processes involved in the development of Neuroscience Centers of Excellence across the continuum of care. The Neuroscience Coordinator is responsible for formulating short term and long term strategies to ensure the organization's vision, mission and values are carried out through the delivery of high quality, cost-effective patient care for the neurosciences service line with a high level of integrity.POSITION EDUCATION/ QUALIFICATIONS:Current RN licensure by Texas State Board of Nurse Examiners.---Excellent customer service skills.---Computer skills required with knowledge of Microsoft Office suite.---Good written and verbal communication skills required.---Bilingual - English/Spanish preferredBachelor's Degree in Nursing preferred.---Graduate Degree in Business, Healthcare Administration, Nursing, or related degree preferred.---3 years of clinical practice preferred---Understands concepts of research and is able to participate in research programs.JOB KNOWLEDGE/EXPERIENCE:Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices---Develops a comprehensive work plan for implementation of the project elements (new and ongoing activities).---Monitor quality utilization, financial outcome indicators, and monitors implementation of data to achieve established targets.---Effectively uses data in decision-making---Actively participate on assigned projects and committees.---Assists in the research and development of new patient care protocols and provides staff support for research activities.---Completes special assignments as assigned.---In addition to the essential job elements and duties listed here, other duties may be assigned or assumed which are within the scope of the employee's skill, expertise and licensure or certification.POSITION RESPONSIBILITES:Oversight of Epilepsy Center, Spine Center and other neuroscience program designation requirementsLeads the Spine Center of Excellence and Epilepsy interdisciplinary subcommittee meetings . Ensures the timeline and key deliverables of the implementation plan are achieved.Works in collaboration with committee chair and multidisciplinary team to establish best practices for the neuroscience programs.Accountable for the key performance measures of the program and implementing performance improvement plans.Responsible for working with sub-committee on development, revision, and implementation of order sets , clinical pathways, policies and procedures, staff education, patient education. Performs updates and reviews as required by regulatory agency.Partners with surgical services, rehab services, medical surgical units, physical therapy, case management and other departments to ensure efficient quality care is provided.Tracks daily census and helps coordinate patient care during the acute hospitalization and post discharge to facilitate a seamless delivery of care across the continuum.Develops staff education related to neuroscience program performance improvement opportunities . Monitors compliance with completing requirements.Promotes evidence based practice by researching clinical practice guidelines and developing policies and procedures as needed.Collects data for neuroscience program registriesProvides follow up phone calls to patients for satisfaction, readmission prevention, and functional outcome scores.Submits data to regulatory agenciesDevelops and participates in community outreach initiatives and service line growth activitiesServes as a navigator to ensure timely and appropriate evaluation, treatment, and education services for spine center patients.Cross trained with other neuroscience programs as needed.LINES OF REPSONSIBILITES:Senior Director of Neurosciences--- Vice President of Nursing--- Assistant CNO--- Chief Nursing Officer
