POSITION SUMMARY:Transitional care encompasses sending and
receiving transfer of patients with complex needs. Performs nursing
activities in the unit where the nurse is the patient's advocate.
Ensures coordination and continuity of care as patient transfers
from the holding unit to a different level of care within the
facility. Answers patient needs by assisting the physician and
person responsible for care.-- Responsible for the delivery of
patient care through the nursing process of assessment, diagnosing,
planning, implementation, and evaluation.-- Responsible for
directing and coordinating all nursing care for departmental
specific patients based on established clinical nursing practice
standards.-- Collaborates with interdisciplinary team to ensure
effective and efficient patient care delivery and the achievement
of desired patient outcomes.---- Supports the organization vision
and mission.-- Utilizes knowledge of patient's age and cultural
diversity into the provision of patient care.-- Contributes to the
provision of quality nursing care through performance improvement
techniques that demonstrate positive outcomes in patient
care.--------POSITION EDUCATION/QUALIFICATIONS:
- Graduate of accredited school of nursing required
- Current registered nurses license in the State of Texas
- Current BLS certification, ACLS certification within six (6)
months of hire.
- --Good written and verbal communication skills required.
- Ability to read, write and speak English required
- Spanish proficiency preferred
- Ability to communicate clearly and concisely with all levels of
nursing, administration, and physicians
- Hospital experience preferred
- Willingness to cross train to like areas
- Willingness to be trained for charge nurse
- Willingness to become a preceptor after two years of nursing
experience--JOB KNOWLEDGE/EXPERIENCE--:Ability to practice nursing
as described by the following definition by the American Nurses
Association (2016): "the essential core of practice for the
registered nurse to deliver holistic, patient-focused care."
Ability to utilize the best nursing process in the provision of
nursing care including but not limited to administering treatments
and medications, interpreting diagnostic tests, formulating a plan
of care, collaborating with other health care providers and
providing education to patients and/or significant others. The
successful candidate will be required to pass competencies for this
position.
- One (1) - two (2) years of continuous acute care experience as
an RN.
- Require constant and frequent judgment and response,
occasionally in response to disasters or emergencies, actual events
or drills.
- Exhibits tact, enthusiasm, and patience.
- Must have excellent customer service skills.
- High degree of competency/experience in general.
- Requires good communication and organizational skills.
- Requires reasoning ability and good independent
judgment.--POSITION RESPONSIBILITIES:--

- Ensures that care is delivered to each patient in accordance
with the Nurse Practice Act of the State of Texas, and ANA
Standards of Clinical Nursing Practice.

department.
- Provides effective and efficient patient care management.
- Performs nursing care using sound judgment.
- Demonstrates strong and effective managerial techniques/skills
in established lines of authority.
- Maintains professional competence by participating in
continuing education and other learning experiences.
- Addresses patient needs during admission to the holding unit
and ensures effective transition to the floor.
- Adheres to hospital/unit policies, procedures, protocols and
guidelines.
- Responds in a timely manner to meet the needs of the patient
and physician.
- Demonstrates ability to adapt to varied age-specific patient
populations.
- Implements physician's orders in a safe and accurate
manner.
- Is able to admit, discharge and transfer patients.
- Maintains knowledge of the patient's status during
hospitalization and works with healthcare team to adjust and
improve approach to recovery. Utilizes critical thinking skills for
appropriate patient interventions.----
- Maintains detailed and accurate records of nursing

- Participates in health/therapeutic counseling,
teaching/emotional support for--patients and significant

- Makes rounds on assigned patients and prioritizes patient's
needs.
- The nurse acquires and maintains current knowledge in nursing
practice.
- Maintains a safe and clean environment for patients and
co-workers.
- Provides and receives reports in a status of patients at the
change of shifts.
- Administers medication and therapies within the scope of safe
nursing practice.
- Formulates, modifies and implements goal oriented patient care
plans according to facility policies.
- Assumes responsibility to report clinical limitations or the
need for assistance to the supervisor.
- Informs the charge nurse of any anticipated and/or actual
issues/problems. .

- Provides direction to non-licensed nursing staff in the
delivery of patient care.
- Anticipates potential problems situations and intervenes to
offset any adverse impact.
- Contacts the organ procurement organization in order to satisfy
regulatory requirements for the determination of medical
suitability for organ, tissue and eye donation; completes
certificate of referral/request and places same in the medical
record.
- Plans and coordinates time schedule with peers to provide
coverage.
- Ensures the distribution and delivery of equipment/supplies as
necessary.
- Builds a positive work environment and exhibits cooperation by
guiding employees to support the mission of the hospital, providing
developmental opportunities and encouraging teamwork.
- Attends mandatory in-service, yearly education, and
departmental meetings.
- Performs other related duties as assigned or requested.
- Supports and promotes excellence in customer/guest
relations.

Program.
- Supports and promotes the organization's Safety Program.
- Supports and promotes the organization's Risk Management
Program.
