POSITION SUMMARY:
The primary purpose of the Patient Care Coordinator under the
direction of the Advanced Care Center Administrator is to be
responsible for the following:
- Providing support and guidance throughout a patient's care
continuum
- Assist the patient with navigating through our network of
physicians, clinics, hospitals, outpatient centers, patient-support
organizations, and other components of our health care system
- Conduct intake and assessment for target group to determine
their psycho-social, housing, and/or resources needed to obtain
maximum benefits from all available sources
- Prepare and empower patients and their family members with
information on their cancer diagnosis and treatment journey.
- Receive referrals from providers, case managers and discharge
planners
- Refer patients to appropriate external resources
- Assist the medical team in educating the patient and family on
resources available concerning treatment and diagnosis
- Document every encounter in the Electronic Medical Record
- Fulfill Commission of Cancer Accreditation and guidelines in a
timely manner
- Attends and may facilitate department meetings, providing input
regarding problem identification and resolution as well as other
survivor patient care activities
- As requested, prepares reports for, and recommends follow up
and process improvement initiatives to COC committee and/or
departments POSITION EDUCATION/ QUALIFICATIONS:
- High School graduate/GED certificate or equivalent is required
- Valid Texas Driver's license
- Driver's license under CLEAR status with the DMV required
- Current BLS certification
- Bi-lingual English/Spanish JOB KNOWLEDGE/EXPERIENCE:
- Two (2) years' experience working with patients in an acute
health care setting preferred
- Experience in working on interdisciplinary teams required
- Staff member must be able to demonstrate the knowledge and
skills necessary to provide care appropriate to the age of the
patients served on his/her assigned unit
- Willingness to be crossed trained to other areas
- Good written and verbal communication; as well as
organizational skills required
- Ability to read, write and speak English, and able to
communicate effectively in Spanish
- Ability to communicate clearly and concisely with all levels of
nursing, administration, and physicians
- Professionalism, good demeanor and presentation required
- Charismatic, engaging personality with good interpersonal
skills is a must
- Good overall clinical skills, ability to work productively and
effectively
- Actively seeks opportunities for professional growth by
attending educational seminars
- Basic computer knowledge, Word, Excel, Cerner EMR, Paragon.
POSITION RESPONSIBILITES:
- Promotes the facility mission, vision and values by effectively
communicating them to others. Considers mission, vision and values
in developing services, standards and practices
- Initiates communication with patient upon receiving a referral
of a patient with a new cancer diagnosis
- Develop, coordinate and provide services to patients and
families who are faced with social, emotional and situational
stressors precipitated by cancer diagnosis
- Develop relationships with the patient and family, regarding
quality care, cost-effective outcomes and enhance the physical,
psychosocial, and vocational health of the patient
- Assist in planning, program development, community organizing,
policy analysis, legislative advocacy, program evaluation,
task-oriented group work, community education, human services
management, and all other assignments given by administrator
- Outlines needs, then researches and identifies resources to
meet those needs
- Provides patients with education, advocacy, and decision making
support related to the impact of medical illness, social needs,
functional needs, or financial needs on healing, recovery from
chronic care, or end of life care
- Initiates and facilitates groups and works with internal and
external resources as to meet the identified patient needs
- Participates in assigned functions to promote the Advanced Care
Center and related services
- Create reporting documents for department use for tracking
quality activities, trends and patterns, and identifying
opportunities for improvement
- Attends and may facilitate department meetings, providing input
regarding problem identification and resolution, and other survivor
patient care activities
- Other duties as assigned
