- Location: La Feria, Texas
- Outpatient Quality Care / Managed Care
- In Office - Outpatient only position
- Monday through Friday 8:00am - 5:00pm
- No weekends
- Low patient panel
- Great Work/Life balance
- Comfortable with use of Electronic Medical Records (EcW)
- Detect and manage chronic diseases
- Develop patient strategy to reduce hospitalization and ER
visits
- Collaborate with Case Managers and Specialty Providers
- Educate patients on the value of compliance
