Family Practice-Without OB opening in Raymondville, Texas.

• Practice Details:
• Successful Primary Care group expanding
• Rio Grand Valley, Texas
• Location: Raymondville, Texas
• Outpatient Quality Care / Managed Care
• In Office - Outpatient only position
• Monday through Friday 8:00am - 5:00pm
• No weekends
• Low patient panel
• Great Work/Life balance
• Comfortable with use of Electronic Medical Records (EcW)
• Detect and manage chronic diseases
• Develop patient strategy to reduce hospitalization and ER visits
• Collaborate with Case Managers and Specialty Providers
• Educate patients on the value of compliance
