excellence. --POSITION SUMMARY:The Behavioral Health Intake
Coordinator is responsible for providing high quality patient care
and displaying excellent assessment skills in the areas of mental
health and chemical dependency.-- The Intake Coordinator will have
experience in Crisis Intervention, triage of patients in crisis,
acting as liaison to physicians requiring information to make
clinical decisions.-- The Intake Coordinator will demonstrate skill
in determining and documenting medical necessity for admission;
have an understanding of TJC and CMS guidelines as they relate to
access to care in a mental health facility.-- The Intake
Coordinator is expected to function well as a member of the
interdisciplinary team and participate in community relation
activities, performance improvement activities and hospital
sponsored events.----POSITION EDUCATION/ QUALIFICATIONS--:--
Master's Degree in Counseling, Social
Work, Psychology or a related field from an accredited college or
university or Registered Nurse (RN) Related experience can be
substituted for degree for a candidate currently enrolled in an
acceptable Master's Degree program. Current BLS Certification CPI
training within 30 days of hire date Must be able to be sensitive
to cultural and bilingual issues Good written and verbal
communication skills required Ability to read, write and speak
English (Spanish helpful)--JOB KNOWLEDGE/EXPERIENCE--:--
The intake coordinator will have

access to care in a mental health facility. The Intake Coordinator

clinical experience in a position primarily responsible for
assessment, crisis intervention and disposition of mental health
patients in crisis. Experience working in a hospital setting or
emergency room and acting as a liaison between physicians a
plus--POSITION RESPONSIBILITES:

practicesDocuments findings accurately and in a timely
mannerAbility to represent the program well in interactions both
inside and outside the hospital

departments

schedulingMust present a professional imageMaintains department
procedures in the following areas: daily census records, inquiry
calls and referrals phone log, and ensures that the level of
treatment and clinical services is appropriate to all patient needs
upon admission.--Ensures a prompt response to patient and referral
needs, provides clinical review of referral packets, and provides a
recommended response to referral sources.Coordinates admissions
into the facility. Coordinates and communicates with all customers
in a professional manner and provides excellence in customer
service.--
- Assists the leadership Team in assuring compliance with
required documentation such as restraint and seclusion, core
measures as it pertains to inpatient mental health, fall risk
scoring, medical necessity for 1:1 observation, patient education
(i.e. fall prevention, smoking cessation, adverse medication
responses) are reviewed for documentation and compliance.
- Works closely with management and intake Team in establishing
communication between all units, departments, and Providers in
carrying out the plan of care and discharge planning within scope
of practice and accordance with Texas Board of Nursing.
- Will serve as a liaison between patients and families to ensure
proper communication of needs or concerns at the time of
admission.
- Works closely with admitting Psychiatrist in receiving and
recording inpatient orders for medications, interventions, and
planning.
- Is part of the risk prevention and compliance team assists in
safety rounds, participates in daily huddles to identify patients
who may be at high risk for suicide, fall, or violent
behavior.
- Participates in Process improvement program using
Plan-Do-Check-Act (PDCA) model.
- Assists with the TMS program, when available.--LINES OF
